Coping with asthma leaves schools gasping | EdNews Parent ...

Castle Rock mom Heather Clark was so concerned that her 13-year-old daughter, Sami, would have an asthma attack at school and that no one would know what to do, she took to sneaking an inhaler into her daughter?s backpack, just in case. She calls it her ?don?t ask, don?t tell? policy.

She didn?t realize that there was no need to treat the inhaler as if it were contraband.

In fact, since 2005, Colorado law has given students with asthma the right to carry their inhalers with them, and to self-administer their asthma medication, as long as they have a care plan on file with the school.

Last year, the legislature amended that law to remove the requirement for the care plan. But students and their parents are required to sign a contract with the school stating that the student knows how to properly administer the medication. Douglas County schools have adopted a policy in line with state law.

?I didn?t know that, but I?m excited to know it now,? said Clark, who also had asthma as a child. But she?s pretty sure that stated policy and actual practice don?t always coincide.

?The school doesn?t allow it,? Clark said. ?I?ve gotten calls saying she couldn?t have it with her. I?m going to print out a copy of this law and make sure Sami has it in her backpack, along with her inhaler.?

Sami says she?s only had to sneak a puff on her inhaler a couple of times in the past two years, but she likes the security of knowing it?s there. ?At school, if you?re having a problem, you have to go down to the nurse?s office. Unfortunately, for me that?s a couple of staircases away. That?s kind of scary, to be honest.?

The law is one thing, reality something else

Sami?s situation illustrates a greater problem confronting Colorado students with asthma, their parents and their schools. The law is one thing. The realities confronting overworked doctors, spread-too-thin school nurses, perplexed school secretaries and anxious parents may be something else altogether.

?This is a topic that just needs more attention,? said Cindy Liverance, vice president of programs for the American Lung Association in Colorado. ?One out of four people in Colorado has asthma. It can be viewed as commonplace. But people can die from asthma. If you don?t access to your medications, or you don?t know how to use them properly, you can end up in the emergency room.?

While Colorado law empowers children to carry asthma medication with them, health educators are now pushing for every school to develop an asthma action plan for every affected student. That action plan would start with a long conversation between the student, their parents, and the school nurse or health care provider. The school should have a list of all medications the child is taking, what the child?s symptoms are, what medications should be used and when, and at what point emergency medical responders should be called.

ASTHMA FACTS

  • A CDC study shows 34 million Americans (1 in 9) have been diagnosed with asthma at some point in their lifetimes.
  • An estimated 22.9 million Americans ? including 427,000 Coloradans ? currently suffer from asthma.
  • Asthma prevalence is highest among blacks (10.2 percent), followed by whites (7.6 percent), and Hispanics (6.8 percent), including Puerto Ricans (14.1 percent) and Mexican/Mexican-Americans (5.8 percent).
  • Nearly a third of all children with asthma will miss three or more days of school this year.

?Does that happen every time for every child? Sometimes not,? acknowledges Kathy Patrick, assistant director of the Office of Health and Wellness for the Colorado Department of Education and a state school nurse consultant.

?I?m coming from the perspective of a school nurse,? Patrick said. ?When you have 4,000 or 5,000 students, how do you get to sit down with a student with asthma and make sure they know how to use their inhaler, make sure they know how to access help when needed? Nurses do what they can with the limited time they have available. But the nurse to student ratios, as they are, it?s unfortunate.?

Doctors, too, are often too busy to insure newly diagnosed young asthma patients get the education they need to stay out of trouble. The youngsters need to be made aware of things that will trigger their asthma, what to do if they start having trouble, and, above all, how to properly use their inhaler.

?If the doctor is well-versed in having these conversations, it takes five minutes tops,? said Eliza Lanman, senior director of programs for the American Lung Association in Colorado. ?But if they?re not used to doing it, these conversations can take longer, and they just don?t see it as a priority.?

As a result, she said, newly diagnosed children with asthma typically end up in the emergency room an average three times in the first year after their diagnosis, because they don?t know how to minimize an oncoming asthma attack.

Understanding proper use avoids devastating results

Sami Clark says she didn?t know how to properly use her asthma inhaler for the first two years she had it. ?Because I was never taught,? she said. ?I finally learned the right way to use it at asthma camp. I sat down with a nurse there, and it took me two days to learn it.?

This lack of education can also lead to situations such as an incident in January in Monument involving two middle school girls. One of the girls loaned her asthma inhaler to the other girl because she thought her friend was having an asthma attack. When school officials found out, both girls were suspended because school policy prohibits sharing of any prescription drug. The girl who borrowed the inhaler was eventually expelled.

?These two girls are a perfect example of why students need to be educated,? Liverance said. ?To think you can just hand an inhaler to a friend is wrong.?

Some Colorado school districts already have asthma policies in place and do a good job of making sure every student with asthma has a care plan on file in the office. Denver Public Schools, in particular, has partnered with National Jewish Hospital to provide asthma education across the district. Liverance cites Cherry Creek and Aurora as other model districts for asthma care.

But she can?t say how many districts have policies in place, let alone how many actually follow them.

?It?s hard to figure out,? she said. ?That?s something we?ve been working on since the bill went into effect last year. It?s in flux. The more districts we talk to, the more interested they are in implementing this. But it not only means more work for schools, it also means more work for doctors, more work for parents. It?s understanding that asthma isn?t something you can take lightly.?

Adopting and implementing asthma care policies isn?t something just limited to large districts. Jody Stroh, the school nurse in Creede, is a certified asthma educator. She?s traveled around southern Colorado training her peers in best practices when it comes to managing students with asthma.

?Our goal is to improve asthma literacy among all the school nurses in Colorado,? Stroh said. ?But I know that it will probably be a school secretary doing the education about asthma with a child, so we?ve designed a file system with all the different worksheets about what asthma is, what medications you?d use, how to exercise without triggering asthma.?

?A tremendous amount of thought has gone into making sure kids have their medicines available to them immediately,? she said. ?We find out who has asthma, who is carrying an inhaler, and those who?ve had any problems in the past year will get a care plan. If you have a child old enough to carry an inhaler, we have a document that we have them sign, and have their parents sign, that says they understand that they?re not to share that inhaler and that they?re must have it labeled.?

New CEO at IBM revives Augusta membership debate

As a club that prides itself on tradition, Augusta National has unwittingly wound up in the middle of a membership debate it thought it was done with nearly a decade ago.

Just seven days before the Masters, no less.

The last four chief executives of IBM ? a longtime corporate sponsor of the Masters ? have been members of the exclusive golf club in Augusta, Ga. The latest CEO of the computer giant happens to be a woman. Virginia Rometty was appointed earlier this year.

One problem ? a woman has never worn a member's green jacket since Augusta National opened in 1933.

"I think they're both in a bind," Martha Burk said Thursday evening.

Burk spearheaded a campaign 10 years ago for the club to admit a female member, applying pressure on just about everyone connected with the club and with the Masters, the major championship that garners the highest TV ratings. She demanded that four companies drop their television sponsorship because of discrimination. She lobbied PGA Tour Commissioner Tim Finchem not to recognize the Masters as part of the tour schedule.

But it didn't work.

Hootie Johnson, chairman of the club back then, said Augusta might one day have a female member, but it would be on the club's timetable, and "not at the point of a bayonet." The protest fizzled in a parking lot down the street during the third round of the 2003 tournament.

Now it's back, and this time it has a face ? Rometty, a 31-year veteran of IBM who has been ranked among the "50 Most Powerful Women in Business" by Fortune magazine the last seven years. Rometty was No. 7 last year.

What's the next step?

Augusta National declined to comment, keeping with its policy of not discussing membership issues. IBM has not commented publicly, and did not return a phone call Thursday night.

"IBM is in a bigger bind than the club," Burk said. "The club trashed their image years ago. IBM is a corporation. They ought to care about the brand, and they ought to care about what people think. And if they're not careful, they might undermine their new CEO."

Augusta has a new chairman in Billy Payne, who ran the 1996 Olympics in Atlanta. When he replaced Johnson as chairman of the club and of the Masters tournament in 2006, he said there was "no specific timetable" for admitting women.

The question was raised at the 2007 and 2010 Masters. Both times, Payne said membership issues were private.

Rometty succeeds Sam Palmissano at IBM, which runs the Masters' website from the bottom floor of the media center. According to a list published by USA Today in 2002, the previous three CEOs also were members ? Louis Gertsner, John Akers and John Open.

As the corporate sponsors became the target, Johnson wound up doing away with TV sponsorship for two years at the Masters to keep the corporate partners ? IBM, Coca-Cola and Citigroup ? out of the fray.

Only IBM returned as a TV sponsor for the 2005 Masters. The others were SBC Communications and ExxonMobil.

Burk said it should not be that easy for IBM to hide if the debate gains momentum.

"What IBM needs to do is draw a line in the sand ? 'We're either going to pull our sponsorship and membership and any ancillary activities we support with the tournament, or the club is going to have to honor our CEO the way they have in the past,'" Burk said. "There's no papering over it. They just need to step up and do the right thing.

"They need to not pull that argument that they support the tournament and not the club," she said. "That does not fool anybody, and they could undermine their new CEO."

Burk said she would not be surprised if IBM pressured Rometty to say she doesn't want to be a member.

"Really, I don't think it's her responsibility," Burk said. "It's the board of directors. They need to take action here. They don't need to put that on her. They need to say, 'This is wrong. We thought the club was on the verge of making changes several years ago, and we regretfully end our sponsorship to maintain her credibility and the company brand.' "

The debate returns just in time for one of the most anticipated Masters in years. Tiger Woods finally returned to winning last week at Bay Hill and is considered one of the favorites, along with U.S. Open champion Rory McIlroy. Eight of the top 20 players in the world ranking have won heading into the first major of the year, a list that includes world No. 1 Luke Donald and Phil Mickelson.

Now comes a sensitive issue that dogged the tournament a decade ago, and might not go away easily.

Augusta National does not ban women. They can play the golf course, but no woman has worn an Augusta green jacket, a status symbol in business and golf. Rometty is said to play golf sparingly. Her greater passion is scuba diving.

She now becomes a central figure.

"We have a face, we have a resume, we have a title and we have a credible reason to do it that doesn't involve Martha Burk," Burk said.

Burk said she is no longer chair of the National Council of Women's Organizations. She had planned to step down until the first flap with the Masters began in the summer of 2002. Now, she said she runs the Corporate Accountability Project for the council, a project born from her battle with Augusta.

World's first bedside genetic test

ScienceDaily (Mar. 29, 2012) ? Developed in Canada and conducted by researchers from the University of Ottawa Heart Institute (UOHI), in partnership with Spartan Bioscience, the world's first bedside genetic test has received acknowledgment by The Lancet.

The article "Point-of-care genetic testing for personalisation of antiplatelet treatment (RAPID GENE): a prospective, randomised, proof-of-concept trial," reports on the use of a simple cheek swab test, the Spartan RX CYP2C19, performed by nurses at the patient's bedside. This revolutionary technology allows doctors to rapidly identify patients with a genetic variant known as CYP2C19*2. Cardiac stent patients with this variant are at risk of reacting poorly to standard anti-platelet therapy with Plavix? (clopidogrel).

The study demonstrated that tailored drug treatment therapy made possible by the genetic testing successfully protected all of the patients with the at-risk genetic variant from subsequent adverse events, while 30 per cent of patients treated with standard therapy did not receive adequate protection.

"For the first time in medicine, nurses were able to perform DNA testing at the patient's bedside. This is a significant step towards the vision of personalized medicine," said Dr. Derek So, Interventional Cardiologist at the University of Ottawa Heart Institute (UOHI), and principal investigator of the RAPID GENE study.

Study Details

The RAPID GENE study enrolled 200 patients who were being treated with cardiac stenting for an acute coronary syndrome or stable angina. Patients were randomized to a treatment strategy of rapid point-of-care genotyping and Effient? (prasugrel) for CYP2C19*2 carriers, or to standard therapy with Plavix? (clopidogrel). The Spartan RX CYP2C19 bedside DNA test was performed by nurses who received a 30-minute training session, but had no prior laboratory training. The test had a sensitivity of 100% and a specificity of 99.4% compared with DNA sequencing. For CYP2C19*2 carriers, treatment with prasugrel completely eliminated High on-treatment Platelet Reactivity (HPR). HPR is a marker for patients at risk of complications after stenting. In contrast, 30.4% of carriers receiving clopidogrel had HPR at 1 week.

'I'm really excited just to show the world that side of me,' Pauly says of 'Jersey Shore' spin-off, premiering tonight at 10:30 p.m. ET/PT on MTV.

After five seasons of "Jersey Shore," we know DJ Pauly D pretty well. He's the ultimate prankster who never beefs with anyone, takes crucial care of his beloved sneaker collection and has a crunchy blowout that defies all laws of physics.

But the Rhode Island kid has many more sides to show us, and he plans to put them all on display in his spin-off show, "The Pauly D Project." When asked what parts of his personality "Shore" watchers are missing out on, Pauly told MTV News' Sway, "Just how much I am all about my home, my family and my friends — you don't really see that."

In the show — which debuts tonight (March 29) at 10:30 p.m. ET, right after the reboot of "Punk'd" — viewers will tag along as Pauly hits Las Vegas for his DJ residency at the Palms Hotel and Casino with his whole crew in tow, including turntable inspiration Biggie, bodyguard Jerry and wingman Ryan. They'll also get to see Pauly go home to visit his friends and family and sign a mind-blowing record deal with the G-Note division of 50 Cent's G-Unit Records.

Oh, and did we mention he also hits the road as the opening act for Britney Spears? Yeah, buddy.

"Now you get to see me take my closest friends on the road with me from Rhode Island, and you get to see Rhode Island," he said. "I'm really excited just to show the world that side of me."

His hookup with 50 is already paying dividends, as the G-Unit General recently tapped Pauly to help out in the first commercial for the rapper's Street King energy drink. Pauly jumped in front of the camera with 50 and legendary comedian Joan Rivers for what the MC said was a "real oddball group" intended to target a wide range of potential audiences.

Don't miss the premiere of ""The Pauly D Project" tonight at 10:30 p.m. ET/PT on MTV, following the reboot of "Punk'd"! For continuing "Pauly" coverage, be sure to check in with MTV's Remote Control blog.

A pizzeria where all the pies are fried

By msnbc.com staff

This is a story about fried pizza. And no, it?s not from the Texas State Fair. It?s from the heart of pizza snobbery: Manhattan.

Today (assuming everything goes well for a restaurant opening -- always a dicey bet), Guilio Adriani opens on the Lower East Side what he says is the world?s first pizzeria where all the pies are fried.

La Montanara by Forcella will be open evenings and late night at 168 Ludlow Street. And it sounds like Adriani knows his prospective customers well.

?Lots of clubs, lots of drunk people on the Lower East Side. We should do good. I hope so,? Adriani, who is from Naples, Italy, said.

The dough is fried, then toppings are added, then the pizza is baked. The resulting crust ?is very light.?

Adriani owns two other pizza restaurants. He added a few of the fried pies to the menu of one last July.

?It was the best seller, so we decided to go with only fried pies? at La Montanara by Forcella he said.

The idea isn?t completely new. It?s done in some restaurants in Naples he said. But Adriani says his execution is different ? specifically because he uses sunflower oil which he says makes for the lightest possible crust.

?Sometimes when you are a chef you try something and it turns out good,? he said of the experiment.

Reached by phone the day before opening, Adriani seemed calm. Perhaps because it?s his third place.

?When you have one (restaurant) you cook mostly. When you have three you control mostly. Depends on what you like,? he said. ?Right now I like control since if you do something and it works you are a success.?

Eater New York has a copy of the menu, which also features a couple of dessert pies with Nutella and some sandwiches ? fried, naturally.

What do you think? Will you give it a try? Share your thougths on Facebook.

How to save Europe's most threatened butterflies

This press release is available in German.

New guidelines on how to save some of Europe's most threatened butterfly species have been published by a team of scientists co-ordinated by Butterfly Conservation Europe. The report covers 29 threatened species listed on the EU Habitats Directive. Each Member State has a responsibility to conserve these species. The new report will provide crucial information on how to achieve this goal and meet their international biodiversity targets.

The report entitled "Dos and don'ts for butterflies of the Habitats Directive of the European Union" was published in the form of an "Applied Conservation" paper in the newly launched open-access journal Nature Conservation (press release on the journal launch). It includes detailed accounts of each species, their habitat requirements and food-plants, as well as a list of dos and don'ts in the management of their habitats.

European butterflies are under huge threat and almost 10% are now threatened with extinction. The European grassland indicator shows that the abundance of 17 characteristic butterflies has declined by over 70% in the last 15 years. The main reasons for the declines are habitat loss and incorrect management. Many habitats are now either abandoned from agriculture, allowing them to become overgrown with scrub, while others are too intensively managed. The new publication is thus a pivotal step to get remaining habitats better managed.

Butterflies are sensitive indicators of the environment and populations respond very quickly to habitat change. Management for butterflies will help ensure the survival of a wide range of other insects, which form the bedrock of European biodiversity. The guidelines will help ensure that European habitats are managed in a sustainable way that helps the survival of humans as well as wildlife.

The lead author, Chris van Swaay of Dutch Butterfly Conservation said "Managing habitats in the correct way is the single most important issue affecting the survival of European butterflies. This is the first time that practical information has been brought together to address the issue. We hope the advice will be taken up urgently across Europe to help save these beautiful species from extinction."

Klaus Henle of UFZ (Germany) who established the new journal in collaboration with Lyubomir Penev at Pensoft (Bulgaria) within the European Union's FP7 project SCALES says "Biodiversity loss is one of the most important topics facing the future of our planet. Our new open access journal Nature Conservation is intended to make scientific information freely available to help conserve nature and create a healthy world for everyone. The journal aims particularly at facilitating better interaction between scientists and practitioners and its major goal is to support synergistic interactions among scientists, policy-makers, and managers."

Arab ministers want Syria to halt crackdown

BAGHDAD (AP) ? Arab countries, divided over how to stop Syria's bloodshed, plan to call for the Syrian regime to halt its crackdown on civilians, let in humanitarian groups and free detainees, according to recommendations Wednesday by foreign ministers preparing for a summit this week.

Even before Arab heads of state began their Thursday summit in the Iraqi capital, Syria sharply rejected any measures they take. A Syrian Foreign Ministry spokesman, Jihad Makdessi, said Damascus would "not deal with any initiative" that might come out of the 22-member Arab League.

The rejection reflected Damascus' refusal to work with the League after the pan-Arab body suspended Syria's membership as punishment for the bloody crackdown on protesters demanding the ouster of President Bashar Assad. The U.N. says that over the past year, more than 9,000 people have been killed in the violence.

Arab countries are divided over how aggressively to intervene in Syria's turmoil. Gulf nations, particularly Saudi Arabia and Qatar, are believed to want to start providing weapons to Syria's rebels and even carve out a "safe area" inside the country that the opposition can use as a staging ground.

Iraq, the host of this week's summit, is more cautious. Baghdad's Shiite-dominated government is close to Iran, Assad's closest ally, and is wary of hurting those ties. On Wednesday, Iraqi Foreign Minister Hoshyar Zebari told Arab foreign ministers gathered in Baghdad that his country rejects foreign intervention in Syria but supports what he called the aspirations of the Syrian people.

"We stress our full support to the aspirations and legitimate demands of the Syrian people to freedom and democracy and their right to determine their future, choose their leaders and the peaceful transition for power," said Zebari, a Kurd.

Zebari sounded more assertive at a news conference, saying Iraq "could no longer remain neutral" in the face of the violence in Syria, its neighbor.

He also said that the Syria crisis was headed toward "internationalization," maintaining that the Arab League already has done all it could to resolve the conflict.

Assad's regime on Tuesday announced its acceptance of an initiative by U.N. envoy Kofi Annan calling for a cease-fire. However, violence continued with clashes between government forces and armed rebels. Syria's opposition is deeply skeptical that Assad will carry out the terms of the peace plan.

Arab foreign ministers on Wednesday were working out recommendations for the heads of state to approve when they meet here Thursday. According to draft recommendations obtained by The Associated Press, the summit would call on Assad to implement a cease-fire, let in humanitarian aid and release all those detained the past year. It also demands he allow peaceful protests, withdraw army troops from urban centers and implement previous Arab League proposals to end the conflict.

Speaking before Zebari, Libya's foreign minister, Ashour Ben Khayil, was much more forceful about the conflict in Syria.

"We witness the tragedy of our brothers in beloved Syria who have, for more than a year, face a tyrannical and harsh regime. Regrettably, the world stands by, unable to take decisive action," he said. "What we witness every day across Syria is genocide ... against a people whose only sin is that they demanded freedom and dignity."

Libya, where longtime dictator Moammar Gadhafi was ousted and killed last year, hosted the last Arab summit, in 2010. Last year's Arab Spring upheavals led to the canceling of the summit, meant to be held in Iraq.

Opposition members accuse Assad of agreeing to Annan's plan to stall for time as his troops make a renewed push to kill off bastions of dissent.

"We are not sure if it's political maneuvering or a sincere act," said Louay Safi, a member of the opposition Syrian National Council. "We have no trust in the current regime. ... We have to see that they have stopped killing civilians."

Annan's plan calls on Damascus to immediately stop troop movements and use of heavy weapons in populated areas and to commit to a daily two-hour halt in fighting to allow humanitarian access and medical evacuations.

Gulf Arab nations, particularly Saudi Arabia and Qatar, have been pushing behind the scenes for more assertive action to end the Syria conflict. Privately, they see little benefit in the Arab League's efforts to reach a peaceful settlement and prefer instead to see a small core of nations pooling together to act on their own.

Among the options they are considering are arming the Syrian rebels and creating a safe haven for the opposition alongside the Turkish-Syrian border to serve as a humanitarian sphere or staging ground for anti-regime forces.

The Sunni-led Gulf countries are hoping that by forcing Assad's fall, they can pull Sunni-majority Syria out of its alliance with Iran and break the belt of Tehran's influence that stretches through Iraq and Syria to the Mediterranean.

A Sunni-ruled Syria would give the U.S.-allied Gulf Arabs a significant victory in their long-running power struggle with non-Arab, mainly Shiite Iran.

In a sign of their cool ties with Baghdad's Shiite leadership, most ? perhaps all ? rulers of the Gulf nations were likely to stay away from the summit and send lower-level officials. Jordan's King Abdullah, also a Sunni, is not attending, sending his prime minister in his place, the government announced.

In a possible breakthrough in Iraq's relations with Bahrain, Iraqi Prime Minister Nouri al-Maliki met on the sidelines of Wednesday's meeting with Bahrain's foreign minister, Sheik Khalid bin Ahmed Al Khalifa.

No details emerged from their meeting.

Relations have been tense since Iraqi politicians publicly criticized last year's crackdown by Bahrain's Sunni-led regime against the nation's Shiite majority.

Shiite-ruled Iraq is hosting the summit to show that it has emerged from years of turmoil and American occupation. But a massive security operation in Baghdad mirrors fears that Sunni militants could try and disrupt the meetings.

Central Baghdad was virtually deserted on Wednesday, with hundreds of heavily armed troops and policemen deployed in full combat gear. The summit venue is a palace once used by Iraqi dictator Saddam Hussein and is located inside the "Green Zone," a highly secure area on the west bank of the Tigris River where the Iraqi prime minister's office and the U.S. and the British embassies are also located.

"Iraq does not search for a role or seek a role," Zebari said in defense of the $500 million cost of preparing for the Arab summit. "Iraq's history is carved in history."
